The following instructions apply to all
new MOS patients.
► 1. SCHEDULING
AN APPOINTMENT
We've made it easy for you to schedule an appointment by calling or
using our online Appointment Request Form.
A.
Calling
The easiest way to make an appointment
is to call our office at 925 939-8585 and tell the receptionist you
would like to schedule a new patient appointment. When you call,
please have the following information available.
- Patient Name
- Name of the referring physician, office address, telephone and
fax numbers
- Diagnosis
- Date of diagnosis
- How the diagnosis was made (physical exam, other)
- What treatment, if any, has taken place
- Over what time period the treatment has occurred
- Patient’s telephone number
- Patient’s insurance information

2. SAVE TIME WHEN YOU
ARRIVE
You can save time when you
arrive by completing our New Patient Registration Packet on our secured
site.

Due to the timeframe involved in processing your online information,
we would greatly appreciate you completing our pre-registration form
and submitting it to us 48 hours in advance of
your appointment.

ADDITIONAL INFORMATION
Specific medical
reports may be needed for review at the patient’s first
visit. If diagnostic reports, x-rays or medical information are available to
the patient, please bring this information with you for review at
your first appointment. Please arrive
20-30 minutes prior to your appointment time in order to complete your
patient
information forms. (In
order to prevent delays in your initial appointment please be sure to
bring your insurance cards with you.)
When scheduling for a child under 18, please note that minors must be
accompanied by an adult. Directions

Physicians wishing to to refer a patient can
fax your patient referral information to 925.933-2912.
Your
patient will also be contacted in order to review insurance coverage
and obtain additional demographic information.
New Patient Insurance Tips
If you are enrolled in a managed care program ( HMO, PPO or POS ), our
services may be covered by your plan. Most
managed care companies offer their programs through MOS for diagnosis,
treatment, rehabilitation and prevention.
Check
with your employer or health plan to see if you have access to
Muir Orthopaedic Specialists as a preferred provider. Also
check with your insurance company for any special instructions or
authorizations. Your insurance company can tell you about any co-pays
or deductibles which are your responsibility. Knowing this
information beforehand will help facilitate your visit.
Following
your insurance company's instructions will minimize your out of pocket
expenses. If you find
your insurance company does not cover treatment, ask what you can do to
obtain this coverage. And please
be sure to write down the name of the person with whom you spoke for
future reference.
As
a courtesy MOS will file your claim with both primary and secondary
insurance carriers. We will work with you to help you receive full benefits
from your insurance and will respond to any requests from insurers for
additional medical information.
Each
insurer has specific requirements that must be met prior to
consultation and treatment. Reauthorizations may be required throughout
treatment.
Your MOS patient assistance specialist can
answer questions about insurance verification and/or prepayment
requirements.
