We at Muir Orthopaedic Specialists
know that your time is valuable. As a new patient to our facility
there are numerous required forms that you will need to complete.
We've made it easy for you when you arrive for your first
appointment by providing you all the initial forms online.
In our New Patient Registration process you
will find a total of 4 forms. The first is an online form and the
last 3 are provided in a 5 page PDF packet. The first 2 ( marked with an asterisk
* ) are mandatory. 
1. New Patient
Registration Form * - This
online form needs to be completed by all
new patients and submitted 48 hours in advance of your appointment.
Once you have completed the form in its entirety, please hit submit
and this form will be sent directly to our secure site.

In order for a physician
to perform a full evaluation, we request that the patient medical
history form be completed in full.
2. Patient Medical
History Form* (2
pages - included in the PDF pack of forms available below) - This form is available in PDF
format. In order to complete this form, you will need to print it
out, answer all health questions and bring it with you to your first
appointment.
The forms below are
optional and should be completed, if applicable to you.
3. The Notice of
Privacy Practices Form (2 pages - included in the PDF
pack of forms available below) outlines the uses or disclosures
of your health information permitted by law.

Protected Health Information Authorization
& Acknowledgement of Receipt of
Notice of Privacy Practices. (1 Page - included in the PDF
pack of forms available below) This form does 2 things;
Section 1 is your acknowledgement that you have received and read
the Notice OF Privacy Practices Document. You should read this form
but not sign the Acknowledgement of Receipt section
until you arrive and can have your signature witnessed by a MOS
representative. Section 2 should be completed if
you, as a patient, want to authorize someone to receive your medical
and billing information. (An example of this could be a
married couple where one spouse wishes to permit the other spouse
access to their medical and billing information).
